1) Enroll and Complete a Baseline Survey at Vivent Health:
- Take a 5-minute eligibility survey
- Sign a consent form and provide ways to contact you
- Complete a baseline survey (approx. 45 minutes) and receive $50 for completing the survey
2) Complete 5 Monthly Surveys:
- Complete one 30-minute survey per month, for five months
- Take surveys on your phone, computer, or at Vivent Health
- Receive $20 for each completed survey
3) Complete surveys at 6, 12, 18, and 24 months:
- Complete one 40-minute survey, once every 6-months
- Take surveys on your phone, computer, or at Vivent Health
- Receive $40 for each completed survey
